Medical Insurance

SBS Creatix is pleased to offer our employees excellent benefit options!

SBS Creatix understands the importance of employee benefits and we extend the ability to enhance those incentives most important to you and your lifestyle! We have designed two comprehensive Medical Insurance plans, allowing employees to choose the option that best suits their needs.

Both PPO plans are administered through Cigna and include Prescription Drug Coverage.

Dental Insurance

Our comprehensive Dental Insurance is a PPO plan which is administered through MetLife.

  • Preventive Procedures (i.e. exams, cleanings) 100%
  • Basic Procedures (i.e. fillings, stainless steel crowns) 90%
  • Major Procedures (i.e. bridges, dentures) 60%

Life Insurance

Life Insurance & AD&D

  • Life and AD&D is Company Paid!!
  • 100% of annual salary to a maximum of $150,000

Voluntary Life Insurance

  • The first $10,000 is Company Paid!!
  • Employees may choose to purchase benefits in $10,000 increments to a maximum of $300,000
  • Proof of good health is NOT required for up to $50,000 in Voluntary Life if elected within 30 days of hire
  • Election of Voluntary Life Coverage for those other than new hires will require proof of good health
  • Employees may choose to cover Eligible Spouses in $5,000 increments up to a maximum of $100,000
  • Employee coverage is required for spouse to elect coverage
  • Eligible children 14 days of age or order, employees may elect coverage in the amount of $2,000 or $5,000
  • Child benefits cannot exceed 100% of the employee’s coverage

Disability Insurance

Short Term Disability

  • Short Term Disability is Company Paid!!
  • Benefits Percentage is 60% of pre-disability earnings
  • Maximum weekly benefit is $1,500

Long Term Disability

  • Long Term Disability is Company Paid!!
  • Benefits Percentage is 60% of pre-disability earnings
  • Maximum monthly benefit is $5,000

Section 125

The SBS FSA Program is administered through Paylocity and this allows employees / participants to set aside pre-tax earnings (resulting in a substantial payroll tax savings) that may be used to pay for qualified medical and childcare assistance expenses.

Paid Holiday and Vacation:

SBS employees who elect the option of PTO (paid time off) will enjoy a combination of 10 paid holidays and 10 paid vacation/personal days for a total of 20 paid days off for a full year of service.

Eligible employees may contribute up to the IRS yearly maximum of their annual salary on a pre-tax basis. Edward Jones / Alerus administer our  401k program. Matching is $.50 on each dollar contributed by the consultant, up to 6% of their salary. Consultants are 25% vested in their matching contributions after one year of service then another 25% per year until fully vested in four years!
