We have designed two comprehensive Medical Insurance plans, allowing employees to choose the option that best suits their needs.
Both PPO plans are administered through UnitedHealthcare and include Prescription Drug Coverage.

Our comprehensive Dental Insurance is a PPO plan which is administered through UnitedHealthcare.
- Preventive Procedures (i.e. exams, cleanings) 100%
- Basic Procedures (i.e. fillings, stainless steel crowns) 90%
- Major Procedures (i.e. bridges, dentures) 60%
Life Insurance & AD&D
- Company paid Life and AD&D
- 100% of annual salary to a maximum of $200,000
Voluntary Life Insurance
- First $10,000 is company paid
- Employees may choose to purchase benefits in $10,000 increments to a maximum of $300,000
- Employees may choose to cover Eligible Spouses in $5,000 increments up to a maximum of $100,000
- Eligible children 14 days of age or order, employees may elect coverage in the amount of $2,000 or $5,000
Short Term Disability
- Company paid
- Benefits percentage is 60% of pre-disability earnings
- Maximum weekly benefit is $1,500
Long Term Disability
- Company paid
- Benefits Percentage is 60% of pre-disability earnings
- Maximum monthly benefit is $5,000
The SBS FSA Program is administered through Paylocity and this allows employees / participants to set aside pre-tax earnings (resulting in a substantial payroll tax savings) that may be used to pay for qualified medical and childcare assistance expenses.
Paid Holiday and Vacation:
SBS employees who elect the option of PTO (paid time off) will enjoy a combination of 10 paid holidays and 10 paid vacation/personal days for a total of 20 paid days off for a full year of service.
Eligible employees may contribute up to the IRS yearly maximum of their annual salary on a pre-tax basis. Edward Jones / Alerus administer our 401k program. Matching is $.50 on each dollar contributed by the consultant, up to 6% of their salary. Consultants are 25% vested in their matching contributions after one year of service then another 25% per year until fully vested in four years.
